Welcome to Morgan Park High School Activity Registration

Welcome to Morgan Park Sports Pre-Participation Registration site. To enjoy the privilege of participation in competitive extracurricular activities at Morgan Park High School, each athlete and parent/guardian must read, understand and adhere to the following rules, regulations, responsibilities and conditions and procedures of the CPS Code of Conduct, Morgan Park Code of Conduct, Review of IHSA Athletic Eligibility, IHSA Sports Medicine Acknowledgement & Consent Form (Concussion, PED & Asthma), Photo Release Form and Northwestern Medicine Consent to Treat and medical release form. 

As part of the following registration process, The Morgan Park Code of Conduct will be signed by the individual athlete and parent/guardian. In addition, as part of the registration process, a current Pre -Participation Physical Examination Form signed and dated by a physician must be on file in the Athletic Office or uploaded to the registration page before a student may begin try outs/practice in any sport. Please remember that according to IHSA rules, physicals that are deemed "eligible" are within 13 months from the date of the examination. (i.e, 4-2-20 issue date = 5-2-21 expiration date).

Please register below, all fields must be completed in order to successfully complete registration, which includes the submission of a current physical. Failure to complete the registration will result in the student athlete not being able to participate in try outs/practices.

All student-athletes participating in IHSA interscholastic sports are required to pay a participation fee of prior to the first IHSA contest. You can pay this through your Head Coach account. No refunds will be issued after the first IHSA contest.

Physical Examination

Students registering for Athletics must have a current physical examination on file in the Activities Office before the student is authorized to participate in practice/tryouts. To ensure clearance for participation, physicals should be completed and turned in to the Activities Office no later than 3 school days prior to the first day of practice/tryout.

Physical Examination is valid for 13 months; a physical examination must be completed on or after April 15 to be valid for participation the following school year. Download Physical Form HERE.
